Testosterone

Testosterone for women

We think of testosterone as a male hormone, but women produce it as well. Testosterone levels decrease gradually throughout a woman's life. In some women they may feel this lower testosterone level by having a lower sex drive. Women who have their ovaries surgically removed may experience this more severely than other women. There is evidence that replacing testosterone can improve low sex drive if it is distressing for a woman. Sex drive is complex in women and testosterone replacement doesn't help everyone. Some women report improvement in their energy levels, mood, brain fog and anxiety but at the moment, there is not enough good evidence to prescribe it for these symptoms. Testosterone works best when you have enough oestrogen, so it is important this is replaced adequately first.

Testosterone in women is replaced through the skin using a gel or cream which you rub in. The gel is made for men so women should use a much smaller amount. The cream is made for women but is not available on the NHS, it is licenced in Australia and only available in the UK on a private prescription. If you decide you wish to start testosterone replacement, the Consultant will help you decide which preparation is best for you.

Testosterone replacement in women for low sex drive is not a licenced medication in the UK and we will discuss this with you before prescribing the medication. The data available shows that testosterone is safe in otherwise healthy women although long term safety data is awaited.

If you take the testosterone as prescribed, then side effects are uncommon. These may include acne, unwanted hair growth and weight gain. Other more serious side effects are rare, if testosterone levels are monitored and remain normal.

Abbey Menopause recommend that we monitor your blood testosterone level prior to commencing testosterone replacement and whilst you are using it. This will also be discussed with you during your consultation. If you remain free from side effects, we recommend you try testosterone replacement for at least 3 months as it can take some time to feel the full effect.

Women experiencing symptoms of reduced libido, changes in sexual function, and reduced energy levels causing distress are usual indications for testosterone treatment in women. This treatment can be utilised after other possible contributing factors are considered.

Women on testosterone treatment report improved cognition, mood, and energy levels, but the evidence is limited.

Many factors contribute to our libido, and reduced testosterone alone may not always cause low sexual desire. Psychological and other physical characteristics, certain medications, vaginal symptoms, and mood disturbance can interfere with sexual function.
